What is freeze-all technology?
A few years ago, in IVF it was considered optimal to achieve synchronous maturation of several follicles, aspirate (ovum pick up - OPU) 10 to 12 oocytes, of which 70-80% are mature (in metaphase II-MII). With the development of embryos up to the 5th day, selection was carried out in the embryological laboratory, because only competent embryos reached the blastocyst development stage. With a fresh embryo transfer (ET), one or two blastocysts were returned with a success rate of about 50% clinical pregnancy rate (CPR). The remaining embryos were frozen and FET (freeze ET) was performed later in the natural cycle with a success rate of 30-35% for CPR. Cumulative success was around 60%.
The simple conclusion then was that "more is not better" (more than 12/15 oocytes). It should be remembered that blastocyst ET is an advantage only for patients with a good prognosis.
What have new technologies brought us?

BETTER IVF RESULTS (FET VS. FRESH)
Several major studies have shown that this technology gives better results - OR 1.38 (OPR, CPR) and does not increase complications (spontaneous abortion) or congenital abnormalities in children. With FET, all perinatal indicators are better than in pregnancies after fresh ET. Only children are heavier - large baby syndrome.
A new philosophy of better IVF/ICSI results has been realised along with new possibilities. More aggressive stimulation of ovulation (SO) is allowed only by an experienced human reproduction subspecialist with the knowledge and ability to apply new technologies. A favourable cumulative success rate in younger women today in LBR is between 60-80%. Such a strategy is applicable only to prospective patients.
